Collaborative Improvement Registration - Step 1

Thank you for your interest in registering for a collaborative improvement project!

 

Instructions

In order to complete this process, please do the following on the registration form:

  • Identify a Project Lead AND a Project Sponsor from your organization. Be prepared to enter both individuals during this registration session.

  • Enter each person’s first and last name.

  • Enter each person’s email address that is used at the center.  Personal, generic email address, such as outlook.com, gmail.,com, yahoo.com, and aol.com, etc. are not accepted.

  • Enter each person’s phone number. 

NOTE: As the person initiating and completing this registration, you will receive an email from UNOS containing a Participation Agreement, which is a legal agreement requiring an e-signature.  The email will include instructions on how to sign and return the document.  This email should arrive in your inbox immediately upon registration submission, but please check your email’s SPAM or JUNK folder, in case you do not receive it.

 

The registration process is not complete until the Participation Agreement has been signed and submitted. You will receive a confirmation email. The Participation Agreement email may be forwarded as needed.
